Medical Form

Wisconsin state law requires a health history for every camper. You’ll find a link to this online form on your registration page.

A parent or legal guardian must complete and sign the form. In case of an emergency, it allows Phantom Ranch to authorize medical treatment and gives medical professionals the information they need.

Phantom Ranch follows Wisconsin regulations and ensures all medical forms are HIPAA-compliant.

Check-Out Form

This form tells us who is authorized to pick up your child from camp, as required by state law. You’ll complete and submit it online during registration. You can update it anytime before camp starts.

If needed, you can also make changes when you arrive at camp.

Camper Information Form

This form lets you share helpful details about your child directly with their cabin leaders. It’s part of the registration process and can be updated anytime before camp begins.
